In today’s regulatory environment, it is sometimes difficult for providers to understand why the surveyor is citing a violation and how to respond to that concern. This webinar will focus on the required components of a plan of correction when a facility is identified to have regulatory deficiencies from a standard, complaint or federal survey. The session will give examples of plans for the frequently identified deficiencies. In addition, we will discuss how to utilize your facility Quality Assurance process to assist you will correction and ongoing monitoring techniques. The QA monitoring process will be explored and examples of monitoring tools will be highlighted.

The medical record of a resident is a reflection of the care provided. As a member of the health care team, the DON wants the medical record to be a true representation of the quality of services the resident received. In order to accurately portray these services, your staff’s charting skills need to be continually fine-tuned. This session will provide the DON/Nurse Manager with tips and guidelines of charting. Documentation principles to support skilled nursing for Medicare PPS are discussed and risk factors for medical review. “Take aways” from this session include a variety of documentation resources including a streamlined interdisciplinary teaching record.

Medical Review has significantly increased in recent months. One of the main issues driving denials is the lack of consistency in documentation between therapy and nursing. This presentation will review the five major areas of “skilling” criteria with a goal of improving daily skilled nursing documentation. This workshop is critical and timely in making sure that your staff is supporting the resident’s skilled stay in the facility. Attendees will be able to apply the nursing/facility process to quality improvement and improve skilled documentation. Now, the next step is the addition of the RAC (Recovery Audit Contractors) to assess the additional scrutiny to the reimbursement practices of the facility.

Understanding Your Five Star Rating

The Centers for Medicare & Medicaid Services (CMS) implemented the new Five-Star Quality Rating System on December 18, 2008. The process has caused confusion and anxiety for many facilities. It is difficult to understand the process by which CMS is calculating the number of Stars. In addition, the staffing calculation is now based on the acuity level of residents. This workshop will give you an overview of the program, provide specific examples of survey and staffing calculations and show you strategies you can put into place in your facility to improve your rating. In addition, new interpretive guidelines have caused facilities to reevaluate their internal systems to ensure compliance and readiness for survey.
