Although the MDS process has been in place for years, nurses still have difficulty with the concept of “working” the RAPs. This workshop will explain how the RAPs fit into the RAI process, how the RAPs are needed to develop the plan of care that should “drive” patient care. The importance of this critical component of the process will be explained. Participants will take away examples of RAPs that are properly written to meet regulatory compliance and that are useful in their facility.

The foundation of nursing practice standards centers on a complex body of knowledge with impact from endless sources of change. Nationally accepted Standards of Clinical Practice include the new guidelines for F309 that was published in Advance copy with an implementation date of March 1, 2009. We will discuss the changes to F309 including pain management and the new Quality of Care investigative protocol with the use of foundational tools in assessing resident care. We will assist your facility with tools to help you “keep your arms” around your nursing practice as it relates to this critical practice. In addition, we will review additional components of F309 related to hospice, dialysis and non-pressure sore areas.

Even though the MDS 2.0 has been used for more than a decade, it poses unique challenges for the activity professionals of long term care facilities. Does the staff incorporate effective RAI systems and processes into daily practice? Is the activity staff knowledgeable and adhering to the CMS regulatory requirements? This session provides an overview of the Standards of Practice examining F248 and F249 with interpretive guidelines as well as explanation of key components from the MDS manual including the RAI components: assessments, RAP and care planning.
